Individual Treatment Plans
Chaddock offers specialized behavioral health treatment for youth through the use of Individual Treatment Plans developed by a Core Treatment Team of professionals. Chaddock's residential treatment programs are individualized, meaning we adapt to a young person's specific needs rather than expecting the youth to fit into a rigid, preset program.
The Core Treatment Team consists of the client's therapist, village manager of the cottage where the client resides, school teacher, client services coordinator, and the associate director of clinical services, who supervises the team. The team's main focus is to identify core issues related to the client's self-reliance, interpersonal issues, family issues, crisis interventions and behaviors, transition needs, psychosocial issues, and case management. Each professional shares information during Core Treatment Team meetings about the client's behaviors in regards to their area to facilitate a collaborative and consistent Individual Treatment Plan (ITP) for each client
Individual Treatment Plans are used by the team for assessment, planning, implementation and evaluation of the client's treatment needs and progress. |
