Adoption Services
Chaddock is licensed by the State of Illinois, as a Child Welfare Agency. Our license number is 004048.
Chaddock is a Purchase of Service agency contracted by the Department of Children and Family Services to provide services for children of which they have guardianship uf. These services may include adoption services as a permanency option for children if reunification with birth parents is no longer a possibility. Chaddock provides foster care conversion adoptions for children and prospective adoptive parents. Chaddock does not accept legal custody of children and thus is not in a position to make the decision in and of itself that the adoption process should be initiated. However, Chaddock does make recommendations to the Department of Children and Family Services as to the appropriateness of beginning the adoption through legal screening procedures.
Additional adoption services include completions of Homestudies contracted with the Department of Children and Family Services or through private requests. These services include in home visitations, interviews with the child to be adopted and other members residing in the home, Child Abuse & Neglect Tracking System search within the State of Illinois, Criminal search with the Office of the Inspector General, Interview with school providers, reference checks, and questionnaire reviews. Families also must complete fingerprint based background check. Chaddock then provides recommendation of service needs, concerns noted and recommendation of adoption if appropriate.
Policy and Practices:
Chaddock solely provides Adoption Conversions contracted service through the Department of Children and Family Services and adheres rules and procedures outlined through the State of Illinois.
General Eligibility Criteria for adoptive parents:
All adoptive parents must meet State of Illinois Licensing Standards for Foster Family Homes which includes the following:
- All members of the household must provide medical evidence that they are free of communicable diseases or physical and mental conditions which affect the ability of the family to provide care.
- Criminal background clearance for all household members age 18 or older and CANTS clearance for all members 13 years or older. Certain criminal convictions prohibit an individual from being licensed.
- Applicants must be single or married.
- Applicants must be 21 years or older.
- Home must meet standards for space and safety.
- Applicants will be required to participate in a training program.
Fee schedule:
All fees for adoption conversion services rendered are provided by the Department of Children and Family Services and not the adoptive parent.
A primary fee of $850.00 is charged for completion of private homestudies within a 30 mile radius of Quincy. For Home Studies beyond 30 mile radius of Quincy, Illinois an additional fee of $35.00 per hour of travel and mileage expenses of $ .445 per mile is charged. Additional fees not included in primary rate could include $50.00 per hour additional fee for court testimony if necessary, fingerprinting fees designated by the state and agency providing fingerprints, medical evaluations if applicable, certified documents as necessary (marriage license, divorce decrees, birth certificates, court orders, etc.), Child Abuse and Neglect searches outside the state of Illinois, if applicable
